About Us
The Emerging Futures Collaborative Limited (EFCL) is a ministry of Catholic Religious Institutes in Australia, established to provide ongoing administration and support services to Religious Institutes (“Service Partners”). One of the services which EFCL provides to its Service Partners is health and wellbeing coordination.
We are seeking a dedicated and compassionate Health and Wellbeing Coordinator to join our multi-disciplined team providing support services at the Sydney site for the Presentation Sisters Wagga Wagga.
About the Role
Reporting to the EFCL Shared Services Manager, and working collaboratively with others, you will ensure a comprehensive suite of care and support services is provided to each Sister, to enable her to live her life to the full and maintain her independence for as long as possible.
Using a case management approach, and working within an up-to-date model of care, you will assist the Sisters to access and transition to new services; ensure health and aged care entitlements are utilised in an optimal way; maintain relationships with preferred home care providers, and oversee their service provision.
You will maintain relationships with residential aged care providers and ensure Sisters in residential care have a contact person for regular support. You will provide individual advocacy and accompaniment to each Sister in a caring and compassionate manner.
Other responsibilities include ensuring each Sister receives appropriate clinical support, for example, by arranging and attending medical appointments, scheduling appropriate immunisations, and organising medical and allied health assessments, such as vision and hearing assessments.
The role is a permanent, part-time role (22.8 hours a week) based in Sydney, with some flexibility required.
About You
You will have a background in health or allied health, be a compassionate and caring person, and have a genuine desire to support the elderly to live valued and meaningful lives.
You will also have an appreciation of and commitment to supporting the ethos, culture, mission and charism of the Presentation Sisters Wagga Wagga.
Specifically, as the ideal candidate for this role, you will meet the following Key Requirements:
Relevant health or allied health qualification, and experience in an aged care, hospital or community care setting
Case management or care coordination experience in the aged care sector, and demonstrated ability to coordinate clinical practice and care to meet individuals’ requirements
Good understanding of the aged care sector, including familiarity with government-funded services and how to identify and make best use of them, and demonstrated capacity to support individuals through various transitions
Experience in implementing and updating systems and processes for service delivery and ability to apply regulations / legislation in relation to clinical risk and quality improvement processes
Strong communication, listening, and interpersonal skills and proven ability to establish respectful relationships with the Sisters, advocate on their behalf and to influence stakeholders to achieve desired outcomes
Demonstrated ability to maintain confidentiality and discretion.
The successful candidate must have a current driver’s licence and access to a fully insured and maintained motor vehicle. The Sisters are located in Sydney’s west, south-west, inner-west and southern suburbs. As the successful candidate, you must be willing to travel to the various locations.
