About Us


The Emerging Futures Collaborative Limited (EFCL) is an innovative ministry established by a collaborative of Catholic Religious Institutes in Australia, to address an emerging need in religious life for transition and support services. The Presentation Sisters Lismore is one of EFCL’s Service Partners.


The Lismore Congregation is comprised of 21 Sisters with differing living arrangements in Lismore and surrounds. As part of their responsibility to provide support services to the Presentation Sisters Lismore, EFCL seeks to engage a Health and Wellbeing Coordinator.


About the Role


Reporting to the Shared Services Manager EFCL, and within a case management framework, you will ensure each Sister receives quality wellbeing, health and ageing care and support services which enables her to live her life to the full and maintain her independence for as long as possible.


Key responsibilities include:


  • assisting Sisters to access health and aged care services and support in an optimal way, including clinical care, wellness and reablement services, domestic assistance, personal care, pastoral care and companionship, social support, transport, aids and equipment

  • ensuring accommodation meets safety standards for residents and as a workplace for carers and staff entering the accommodation

  • provision of group and individual programs to promote health and wellbeing

  • advocating on behalf of and accompanying the Sisters to medical appointments

  • maintaining an up-to-date model of care and suite of operating policies and procedures for each model of care accountability

  • ensuring health records are maintained and up to date

  • maintaining relationships and agreements with preferred Home Care providers and overseeing their service provision

  • maintaining relationships with preferred residential aged care providers and ensuring Sisters in residential care have a contact person in place providing regular support.


The role is a permanent, part-time role with the successful candidate required to work 22.8 hours every second week on Tuesday, Thursday and Friday (that is, one week on and one week off). The role is shared with another Health and Wellbeing Coordinator who works the alternate week.


About You


Your experience in case management and/or care coordination in an aged care setting is key to this role, as is your genuine desire to support elderly people to live valued and meaningful lives.

Specifically, as the ideal candidate for this role, you will meet the following Key Requirements:


  • Relevant health or allied health qualification with experience in an aged care, hospital or community care setting (Note: Extensive experience in an aged-care setting is preferred and relevant qualifications in social work, nursing or community care will be favourably considered)

  • Effective communication, interpersonal, advocacy and stakeholder-management skills and demonstrated ability to handle sensitive inquiries with tact, discretion and confidentiality

  • Appreciation of and commitment to supporting the ethos, culture, mission and charism of Australian religious institutes

  • Good understanding of the aged care sector and familiarity with government funded and other services, so as to support the Sisters through various transitions, such as sourcing care packages

  • Demonstrated awareness and understanding of clinical and social issues experienced by older people and ability to coordinate clinical practice and care to meet each Sister’s requirements

  • Experience in implementing and updating policies, systems and processes for service delivery and ability to apply regulations / legislation in relation to clinical risk and quality improvement processes


The successful candidate must have a current driver’s licence and access to a comprehensively insured motor vehicle.


A National Police Check, up-to-date Covid-19 vaccination status (as determined from time to time by the Commonwealth Government), and annual flu vaccine are also required for this role.
